Indication ในopen reduction of condylar fracture

1. Displace into glenoid fossa 2. Lateral displacement 3. Foreign body into joint space 4. มีการ overlap of segment > 5 mm 5. หักทำมุมมากกว่า 30 องศา 6. Bilateral condylar fracture 7. Multiple facial one fracture

มักเป็นmixed radiolucency-radioplaque

Tetanus

สิ่งที่เรากลัวคือ
-แผลบริเวณใบหน้า ใกล้ nerve distribation มีโอกาสติดเชื้อได้สูง
-เรากลัว rabies > tetanus

ทำไม?
- ระยะเวลา เด็กส่วนใหญ่ มักได้รับวัคซีน ตั้งแต่2 ขวบ ถ้าได้ในระยะเวลา 5-10 ปี แผลสะอาด คงไม่ต้งให้

Protocol tetanus
- ไม่เคยรับ วัคซีน จะแผลสะอาดหรือสกปรก ต้องได้ TT and TAT
- ได้รับน้อยกว่า 5 ปี แผลสะอาดหรือสกปรก ให้ TT
- ได้รับระหว่าง 5-10 ปี ให้แค่ TT
- ได้รับมากกว่า 10 ปี แผลสกปรกให้ทั้ง TT and TAT


Dose Tetanus
-TT 0.5 ml im
-TAT : prophylaxis 250 iu, treatment 3000-6000 iu

Dose vaccine
-Eric 40 u /kg

Analysis Chin in AP position

NB line : เป็นระยะทางที่ลากจากNB ลงมา แล้ว จุดที่ตรงกับ lower and Pog จะอยู่ในค่าเฉลี่ยน 4-6mm)

APo line : เป็นระยะทางจาก A-Pog line  ถึง lower incisor ค่าปกติอยู่ที่  2mm +/- 2mm

Angle of facial convexity : เป็นมุมที่เกิดขึ้นระหว่าง Ga'-Sn กับ Sn-Pog' โดยค่าเฉลี่ยที่  11 +/- 4 องศา

 Subnasale vertical : เป็นระยะทางระหว่าง soft tissue chin ถึง เส้นที่ลากผ่าน Sn ไปตั้งฉากกับ FH plane ค่าปกติจะอยู่ระหว่าง -3mm +/- 3mm

O degree meridian : ระยะห่างระหว่าง soft tissue chin  ถึง เส้นที่่ลากผ่านNa ไปตั้งฉากกับ FH plane ปกติ จะอยู่ระหว่าง 0mm +/- 2mm

E line : ระยะทางระห่าง ริมฝีปากล่าง กับ esthetic plane โดยจะลากผ่านปลายจมูกกับsoft tissue chin โดยปกติ เฉลี่ย 2mm +/- 2mm

Z angle เป็นมุมทีเกิดจาก FH plane  ทำกับ เส้นที่ลากผ่านsoft tissue chin กับ lower lip ปกติ 68 องศา
