Graft classification
1.Split thickness skin graft จะรวมชั้นของ epidermis และบางส่วนของdermis แบ่งออกเป็น

• Thin (0.005-0.012 inch)  = 0.127 - 0.305 mm
• Intermediate (0.012-0.018 inch) = 0.305 - 0.457 mm
• Thick (0.018-0.030 inch) = 0.457 - 0.762 mm

2.Full thickness skin graft คือ เอามาทั้งepidermis and dermis
3.Composite graft จะรวมทั้งskin cartilage or other tissue


Healing of skin graft

1. Plasmatic imbibition (drink plasma)
2. Capillary inosculation จะมี new blood vessel จากrecipient วิ่งเข้าไปที่graft จะเกิดขึ้นภายใน 36 hr
3. Revascularization 

Graft survival 

• Adherence จะ ติดทันที และจะ สูงสุดใน 8hr 
• Revascularized graft จะเกิดขึ้นใน 2-3 day of post grafting 
• Full circulation to the graft restored in 6-7 days of post grafting

Wound contraction จะเริ่มขึ้นทันทีหลังจากtake graft และจะเกิดไปเรื่อยๆอย่างช้าๆ กินเวลานาน 6-18เดือน 

• FTSG = least contraction
• Thick STSG 
• Thin STSG
• Open wound = most contraction

Note 

• Hair follicle จะไม่มากับ STSG
• Sensation จะกลับมาในเดือนแรก แต่จะไม่ทั้งหมด STSG จะเร็วกว่า แต่ FTSG จะสมบูรณ์กว่า
• Graft ไม่ควรโดนแดด อย่างน้อย 12 month เพื่อป้องกันhyperpigmentation

Graft failure

• Harmatoma
• Seroma formation 
• Movement or shear force
• Poor recipient site
• Poor vascularization
• Contaminated site
