Definition
- Jugulodigastric node: Sentinel (highest) node จะพบอยู่จุดสูงสุดของ Internal jugular chain ซึ่งจะอยู่บริเวณของangle of mandible
- Virchow node : Signal (lowest) node จะพบอยู่จุดต่ำสุดของ Internal jugular chain โดยจะอยู่ในsupraclavicular fossa
Nodal level of neck ประกอบไปด้วย
- Level I เป็น Submental and submandibular group แบ่งเป็น
- IA : Submental node จะอยู่ระหว่าง anterior belly of digastric และ midline ::Receive from chin, lip, FOM, ant tongue Drain to Submandibular node
- IB : Submandibular node จะอยู่แถวๆ subman gland ::: Receive from chin, lip, FOM, ant tongue Drain to internal jugular chain
- Level II, III, IV เป็น High, middle, low internal jugular chain จะอยู่ตามความยาวของ internal jugular vein โดยวางตัวอยู่บน carotid sheath
- II : High jugular node : ตั้งแต่post belly of digastric m. ถึง hyoid bone
- III : Middle jugular node : Hyoid bone ถึง cricoid cartilage
- IV : Low jugular node : Cricoid cartilage to clavicle
- ทั้งหมดจะdrain ลงsubclavian vein or internal jugular vein
- Level V จะเรียกเป็น Spinal accessory chain แบ่งเป็น VA and VB จะวิ่งไปตามแขนงของ spinal accessory nerve โดยส่วนบนสุดของchainนี้จะต่อกับ jugulodigastric node
- VA : High : mastoid to cricoid cartilage
- VB : low : cricoid cartilage to clavicle
- จะรับจาก mastoid node, occipital node and parietal scalp and lateral neck
- Level VI จะเรียกว่า Anterior cervical group จะมี 3 subgroup
- Prelaryngeal chain (Delphian chain) รับจากsubglottic larynx and thyroid ,drain ลง sup mediastinum
- Pretracheal chain อยู่ตามแนวของexternal jugular vein บน superficial fascia นอกต่อstrap muscle รับจากdermal lymphatic ที่anterior neck
- Paratracheal chain ตามtracheoesophageal grooveใน visceral space รับdrainจาก thyroid, esophageal and tracheal
- Parotid group จะรวมทั้งนอกและใน gland และอยู่รอบๆfascia ที่parotid space
- Receive drain from ext auditory canal, eustachian tube, skin รอบๆนั้นแหละ , drain into high jugular chain
- Retropharyngeal group
- Facial group
- mandibular node :ตามขอบของmandible
- Buccinator node : ใน buccal space
- Infraorbital node : ในnasolabial fold
- Malar node : malar eminence
- Retrozygomatic node : deep to zygomatic arch
